FMAE Summer Generative Theatre Camp
Dear Parent/Guardian: Welcome to the FMAE Summer Generative Theatre Camp! To ensure the safety, health, and enjoyable experience of every camper, please read this contract carefully in its entirety and sign to confirm your agreement. If you have any questions, please do not hesitate to contact us.
1Camp Information

Daily Hours:9:00 AM — 4:00 PM
Duration:2 Weeks (10 consecutive days)
Ages:5 – 14 years old
Age Groups:Group A: Ages 5-8 / Group B: Ages 9-14
Language:Bilingual environment (English primary)

This camp is led by professional drama education instructors who use generative theatre methods including improvisation, role-play, and physical expression to inspire children's creativity, communication skills, and self-confidence.

2Fees
Tuition:$1,500 (2-week session, includes all course materials and activities)
Payment:Bank Transfer / Zelle / Cash (please contact us in advance to confirm)
⚠ Payment Notice: Full tuition payment is required to secure enrollment. Once confirmed, the tuition is considered as reserving your spot. For refund requests, please refer to Section 7 Refund Policy.

Tuition includes: All drama instruction, rehearsal materials and props, and daily camp activities.

Lunch: Parents are responsible for preparing lunch for their child each day. Children should bring their own lunch to camp. No lunch service is provided.

3Parent Responsibilities
  1. Pickup & Drop-off: Parents/guardians must drop off children at camp by 9:00 AM each day and pick them up promptly at 4:00 PM. If someone else will pick up your child, please notify camp staff in writing in advance.
  2. Lateness: If a child arrives more than 15 minutes late, parents must inform and register with camp staff. Arrivals more than 30 minutes late will affect the child's participation in daily activities.
  3. Stay Reachable: Please ensure the emergency contact number provided remains active throughout the camp period.
  4. Clothing & Supplies: Please prepare comfortable athletic clothing, sneakers, a water bottle, and any other items specified in camp notifications.
  5. Lunch: Parents are responsible for preparing and packing lunch each day. Clearly label the lunch box with your child's name. Camp does not provide lunch service.
  6. Respect Camp Operations: Parents should respect camp scheduling and safety management. Do not enter teaching areas and disrupt normal activities.
  7. Information Updates: If contact information or health status changes, please notify camp immediately to update records.
4Student Code of Conduct (Camp Rules)

To maintain a safe and pleasant learning environment, all campers must follow these rules:

  1. Respect Others: Respect teachers, staff, and every camper. Use polite language. Insulting or aggressive language is prohibited.
  2. Follow Instructions: Listen carefully to teachers and staff, and participate actively in class activities.
  3. Safety First: Stay within designated camp areas. Do not leave without permission. Follow safety rules during games and activities.
  4. Care for Property: Take care of camp facilities, props, and materials. Intentional damage is not tolerated.
  5. Zero Tolerance for Bullying: Any form of bullying, exclusion, or harassment will result in notification to parents and possible dismissal from camp at the camp's discretion.
  6. Personal Belongings: Campers are responsible for their own personal items. The camp is not responsible for lost or misplaced items.
  7. Electronic Devices: No phones, tablets, or other electronic devices without prior permission.
⚠ Disciplinary Action: The camp reserves the right to suspend or terminate enrollment for campers who seriously violate camp rules or fail to respond to repeated warnings. Refunds for dismissed campers will be handled according to Section 7 Refund Policy based on actual attendance.
5Safety & Health Provisions

🩺 Health Information Disclosure

Parents must truthfully complete the following health information. Omission or concealment may compromise your child's safety:

  1. Medication Management: If your child needs to take medication during camp, please hand the medication to camp medical staff and complete a medication instruction card (including drug name, dosage, and timing). Camp staff will not administer medication on their own.
  2. Emergency Plan: The camp is equipped with a first aid kit and staff hold first aid certification. In emergencies, we will contact 911 and the emergency contact immediately.
  3. Infectious Disease Prevention: If your child has symptoms such as fever, vomiting, or diarrhea, please keep them home and notify the camp. The camp reserves the right to refuse admission to sick children for public health reasons.
  4. Outdoor Activity Safety: Most camp activities are indoors, but some may take place outdoors. Please ensure your child wears appropriate footwear and clothing.
  5. Food Safety: Children bring their own lunch. Please clearly label any food containers containing allergens. Camp staff will be alerted to allergies, but parents bear primary responsibility for the safety of their child's brought lunch.
6Privacy & Photo Authorization
  1. Personal Information Protection: The camp will keep all personal information submitted by parents and children (names, contact information, health information, etc.) secure. It will only be used for camp operations, safety management, and emergency contact, and will not be sold or shared with any third party.
  2. Photo/Video Authorization: The camp may take photos and videos during activities for documenting camper growth, camp promotion, and social media posting. By signing this contract, you consent to the camp using photos and videos of your child for non-commercial purposes.
  3. Privacy Opt-Out: If you do not wish for your child's photos or videos to be used for any public purpose, please note this when signing, and we will ensure your child's image does not appear in public materials.
  4. Data Retention: The camp will retain your information for a reasonable period after the camp ends, after which it will be securely destroyed. You have the right to request deletion of personal information at any time.
I consent to the camp using photos/videos of my child in promotional materials and social media (non-commercial use only).
I do NOT consent to any public use of photos/videos of my child. Please ensure their image does not appear in public materials.
7Refund Policy
  1. 14 or more days before camp starts: Full refund minus a $50 administrative fee.
  2. 7 – 13 days before camp starts: 50% tuition refund.
  3. 6 days or less before camp starts: No refund, but transfer to another child is allowed (with 3 business days' notice to camp and a $30 transfer fee).
  4. After camp begins: If a child withdraws for personal reasons, no refund will be given for days already attended. If the camp cancels activities, a prorated refund based on unattended days will be issued.
  5. Weather / Force Majeure: In case of extreme weather or force majeure that causes camp closure, a prorated refund or deferred participation option will be offered.
⚠ Note: All refund requests must be submitted in writing (via email) to the camp director. Refunds will be processed within 14 business days of receiving the request.
8Liability Waiver & Limitation
  1. General Waiver: Provided the camp has fulfilled its reasonable safety obligations, parents agree and acknowledge that the camp is not liable for minor scrapes, falls, or similar incidents that may occur during normal participation in drama classes, games, and outdoor activities.
  2. Personal Property: The camp is not responsible for loss or damage to campers' personal items (clothing, water bottles, electronic devices, etc.).
  3. Health Risks: If parents fail to truthfully disclose their child's health conditions or special needs, any resulting consequences are the parents' sole responsibility.
  4. Third-Party Liability: The camp is not responsible for any incidents that occur during travel to/from camp or outside camp premises.
  5. Force Majeure: The camp is not liable for breach of contract resulting from natural disasters, epidemics, government orders, or other force majeure events.
  6. Governing Law: This contract is governed by the laws of the State of California. Any disputes shall be resolved through friendly negotiation; if negotiation fails, the matter shall be subject to the jurisdiction of California courts in the camp's location.
9Emergency Medical Authorization

I hereby authorize the staff of FMAE Summer Generative Theatre Camp to make the following decisions on my behalf in the event of an emergency medical situation where I cannot be reached in time:

  • Call 911 for emergency medical services
  • Transport my child to the nearest medical facility for emergency treatment
  • Consent to emergency medical measures as necessary (including but not limited to CPR, use of an AED, etc.)

Medical expenses incurred from emergency treatment shall be the responsibility of the parent/guardian.

I, the undersigned, confirm that I have carefully read and fully understand all terms of this contract, including the Safety & Health Provisions, Liability Waiver, Refund Policy, and Emergency Medical Authorization. I agree to comply with all terms above and authorize FMAE Summer Generative Theatre Camp to manage and serve my child in accordance with this contract.
